Any thoughts on what it could be?

You don't happen to use cast iron cookware, do you? When I used to
cook in a cast iron frying pan a lot, my blood would always sink
like a stone in the iron test they give you before you donate
blood.

About 1 in 200 people, especially those of northern European descent,
have a genetic predisposition to store too much iron. This is called
hemochromatosis, and the excess iron must be removed by removing blood
like a blood donation, fairly frequently.
