Have you tried the old standbys - JC Penneys and Sears? I know
Penneys does custom shirts, and wouldn't be surprised if Sears did
too. My husband is very tall (6'5") and thin, but his shoulders are
broad enough that when he had his Army dress blues made they had to
start with a size 44 Tall jacket and then almost completely remake it
- took the pocket flaps off to move them, had to cut the collar area
down and slim the sleeves down a bit as well. He can get custom made
shirts from Penneys though, so perhaps they can help you as well.
