What negative medical concerns are those?

As Chris wrote, creatine monohydrate occurs naturally in red meat. By
eating enough red meat, one can very easily (if not economically or
with dietary variety) saturate one's muscles with creatine just as one
can by using supplemental creatine. There's no magic there.
