Beer-belly-guy was a not so fat guy who got fat as an adult. Fat-all-over-guy or fat-pear-guy has been fat from a young age. Speaking as a formerly not-fat-guy, then adult beer-belly-guy, past a point, you do start to get fatter all over but I never had had that much fat on my arms and legs even at my heaviest.
